Submitting Music
If you are a musician and wish to donate CDs to our growing music library, please send them to us at the following address and thank you for your generous support:

Please print and complete both forms to send in with the CD: Authorization Form and Artist Release Form.

We only need one Artist Release Form but will need an authorization form for each CD you submit at a time. If you are sending in several at one time, you only need to fill that out once by listing each on the form.
